About the Event
Supporting Your Child’s Medical Care from Pediatrics Through Transition
Helping Families Build a Coordinated Medical Team for Adulthood
PRESENTER: CLAIR COOK
University of Illinois Chicago
Division of Specialized Care for Children (DSCC)
What happens when a child with complex medical needs moves from pediatric to adult healthcare?
For many families, this transition creates a serious gap in care. Pediatric providers may know a child’s extensive medical history intimately, while new adult-care providers are often unfamiliar with the full picture. Important details, established treatment plans, and ongoing concerns can be lost along the way.
Join us to learn how the University of Illinois Chicago Division of Specialized Care for Children (DSCC) helps families coordinate complex medical care and prepare for the transition to adult services.
Clair Cook will discuss:
Services and resources available through DSCC
Ways to organize and communicate complex medical histories
How to prepare for the transition from pediatric specialists to adult providers
Strategies for building a coordinated medical team that understands your family member’s needs

Event is FREE, but advanced registration is required.
Educational Outreach events are designed to provide families, self-advocates, educators, and service providers with useful information related to the transition process as well as life in the community after educational entitlements end.
